Really confused with DS-3025 VS I-693A

I found a thread on this site which says

"If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693."

My medical along with vaccination were done in India prior to my K1 visa interview and I was given a brown envelope which I handed over to custom office at POE.

I was given original DS-3025 as my vaccination records from Indian hospital where my medical was done.

I submitted copy notarized of DS-3025 with my AOS application.

I didn't receive any RFI yet and got a letter with interview date for AOS next month and it says Bring completed medical examination(I-693) and vaccination supplement in a sealed envelope.(unless already submitted)

Since I have not received any RFI after submitting copy of DS-3025 with my AOS application and I got interview date should I think DS-3025 is accepted instead I-693A?

DS-3025 is a vaccination supplement? It was not given to me in a sealed envelope so what should I do?

Some other threads say that its required to transcribe DS-3025 in to I-693A so am really confused.

Is it must for me to transcribe DS-3025 in to I-693A?

Which Civil Surgeon is cheapest to do this?

I saw a post from another member where they got the same letter asking her to bring in the same medical documents to her AOS interview. She didn't have anything because it was all inside the brown envelope. She went to the AOS interview and told the person conducting the interview the same thing and he just said "ok" and approved her.

I think that as long as your medical was done within a year, you're good to go.

07/20/11 - Interview - passed

07/20/11 - Oath ceremony - same day as interview

Note the 693A has been replaced by a newer I-693 that only the vaccinations part needs to be done by US Civil surgeon.

Is now a US Citizen immigration completed Jan 12, 2012.

Well thanks for your response but some people strongly say that its not necessary to transcribe DS-3025 ( foreign vaccination record) in to either I-693A or newer I-693 and you are saying its necessary...that's what my confusion is.

What I am understanding is that if your DS 3025 has the box 'vaccination completed' checked, you don't need to do anything, you can mail a copy of the DS 3025 though the original should be in the envelope you gave the IO at POE.

Check your copy of the DS 3025 which box is checked. If vaccination were completed, take the copy to the interview and wait. If you get a RFE at the interview then you have to act.

I only sent copies of the DS 3025 for my husband and his kids with AOS application. No RFE yet, case transferred to CSC.
