Told at AOS interview I need I-693 and new vaccinations:

Hi,

I recently went for my AOS interview in San Jose. It was all fairly straightforward until the interviewer asked about my vaccinations and told me I would need more but not what these were. I informed him I was fairly sure that I've had all the vaccinations I needed so he said he'd look into it and contact me by mail.

I had my original medical in Knightsbridge, London on the 11th of April 2008. They told me which vaccinations I needed and these I got at my local GP with the vac. and doctor's details all signed on a sheet. I flew in to JFK last summer, my flight changed at Dublin airport and there they took my brown envelope as they have US customs in the Dublin airport! I am doubtful if the envelope ever made it to the states at all.

With my AOS application I submitted copies of my DS-3025 from my London medical and my additional vaccinations sheet from my GP.

Yesterday I received a notice in the mail stating:

"A decision cannot be made until you submit the document(s) or information requested below. PLEASE MAIL the document(s) information to the above address within 30 days. Pursuant to 8 CFR 103.2(b)(8), you have 4 weeks from the date of this notice to comply, failure to do so may result in the denial of your application."

Under a title saying submit the following documents these two lines were checked:

"X Form I-693 Medical Examination AND Supplemental form to I-693 (all required Immunizations/Vaccinations)"

"X Other - An updated vaccination is required. Send it within a sealed envelope."

Now my questions are:

WHAT vaccination do I need?

I don't have much spare cash at present so would Dr Arnold (I've seen him mentioned on this site a lot) be best for transferring my DS-3025 onto a I-693?

Who do I address return letter to? The officer who 'helped' me or for the Field Office Director who is listed at the bottom of my very uninformative letter?

Any help would be hugely appreciated.

Vaccinations

Q: What are the new vaccination requirements?

The CDC has revised its vaccination requirements. As of July 1, 2008, the following additional vaccinations are required in order to adjust status to legal permanent resident:

Rotavirus

Hepatitis A

Meningococcal

Human papillomavirus

Zoster

NOTE: Some of these vaccinations are required for certain age groups only. During the examination with the civil surgeon, he or she will review the vaccination history, and may determine that certain vaccinations are not necessary or not appropriate. Specific information on vaccines, including tables on age-appropriate vaccines and ACIP recommendations, are available at CDC’s National Center for Immunization and Respiratory Diseases (NCIRD) website, accesible in the related links section of this page.

Thanks for the reply. I found that page on the site too, my medical exam was actually before the 1st June 2008 which means the vaccinations I got at my medical should be sufficient. It was this I don't think my interviewing officer understood. So if I get my details transferred onto a I693 I'm hoping that should be sufficient.
