As to the statement on the form:

"I derive an annual income of_____"

Should I put my 'average' here, say of the last three years? Or, simply go with what I earned in the 2008 year?

With respect to the letter from my boss, should it mention commission sales, or just the hard number from 2008?

The stated CURRENT annual income, in your case, should be a realisitic projection of what you will earn in 2009, this should be supported by a letter from your company, it does not matter if he mentions commission sales as long as it verifies your income, so be sure you coordinate this with him. Best is to write the "boss" letter yourself, give it to him to type on company letterhead and sign. He should be verifying CURRENT GROSS (2009) income. You already have your tax return to verify 2008. Whether it will be the same or not is your projection.

The boss letter should also state when you were hired, that the job is permanent (or not) and that the job is full time (or not) attach some recent (maybe for the two months prior to the interview) check stubs to support your claim.

I would list your average income for the past 3 years, IF you can submit supporting documents, AND you have been working for the same company. This is just my opinion. :) Exactly why I got out of sales 3 years ago. Ramen noodles one month and caviar the next...lol
