I wanted to clarify if the new I-134 form Form OMB No. 1615-0014; Exp. 05/31/10 needs to be notarized? I did not see a section for the notary on the new form. I got the form from the link from http://www.visajourney.com/forums/index.ph...mp;page=k1forms

Just to add more information , I guess I am just answering my own question :

You must sign Form I-134 in your full name. (Note: Signing Form I-135 is under penalty of perjury under U.S. law). For this reason, it is not necessary to sign Form I-134 before a notary, nor to have your signature notarize after you sign it.

I am just worried that the consulate will say it needs to be notarized.

Packet 4 from Guatemala said it needs to be notarized, although I'm pretty sure the instructions are outdated because the available hours for one of the approved doctors had changed too (we found out from the doctor). Since the instructions specifically say to do so and I can get something notarized for free at my bank, I am going to get it done wherever they can squeeze their little stamp. Better safe than sorry, if it's not that difficult in the first place.

The Instructions still for the I-134 still say it needs to be notorised , but there is no where on the I-134 to have it notorised . But i had ours notorised even though there was no room on the i-134 to have it done . And the court house wouldnt notorised it , but i showed the instructions to the notory of public where it says it needs to be notorised . And all the notory did is write signature with a line above it and date with line above it was sworn too me at with line above that and my commishion expires on with a line above that and filled in all of that . She then stamped the i-134 with the seal then i signed and dated it . I personally do not know if the usembassy would have a problem with it not being notorised , but i did'nt want to take the chance . And have to wait longer on administrative review . By sending the i-134 again too usembassy with it being notorised

I also came across this situation on the Ankara Turkey embassy site.

Said the I-134 had to be notarized, but the new I-134 says it does not need to be.

When I sent an email to them to clarify it, they said, "If you use the old form, then yes, it needs to be notarized, but the new one does not."

So, always use the latest forms and check with the embassy if you still have questions.

on a side note, one of the people at my company is a notary public, so I had every piece of paperwork for my I-129F notarized. Doesn't hurt anything, and my petition is running through pretty fast, maybe for that reason?

Who knows? I'm waiting for the "Time-Life" book to come out on this subject :jest:

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.
