Hello all,

i just have a few issues in regards to DS form 156, if anyone could tell me what to put in the item 22: when do you intend to travel to the U.S? I do not have an exact date yet, so what should i put? should i giva rough estimate? or leave it blank? please help :(

Also when i tried to press continue, it said there was an issue in regards to item 8: other surnames used. as i dont have any other surnames, i put N/A because if i put none, they might think that none is another last name, again please help as i am in the middle of completing the forms needed.

thanks for any feedback

Item 22: "Upon visa approval"

Item 8: "none" They will NOT think "none" is another last name, and even it if WAS, you have answered the question correctly. Who's on first? What's on second and NONE is on third! :)

Ok thanks for the feedback but it looks like the forms might have changed since you guys filled them out. On the DS 156 form item 22: when do you intend to arrive in the U.S...... there is no physical way to write upon visa approval or any words for that matter, the only option is to put in a day, month and year. so again, i have no exact date, so im wondering what to put.

Also to throw it in there, im having trouble with the form DS 230 Part 1.....item 20 for example.....list below all places you have lived...etc. Im living at the same address from when i was 3 years old, so in the part living there from/to it wont let you put for example, 09-1986 - present, again you have to put in a month and year, which seems stupid as i am still living here at the same address and will be until i move to the U.S.A , so what the hell do i put there, is it me just having trouble with these forms????

Thanks for any help, it is much needed and much appreciated
