Need help with the form that tells student loans company I am leaving!

I am filling out this form I found on this site page...http://www.slc.co.uk/thresholds/index.html and as always I have got myself stuck

and I am hoping there are those out there who filled this in before me?

First off the national insurance number, because i will post this before i leave should i put it in?

then for the employment boxes a or b I was going to tick b unemployed (in reciept of thrid party financial support) Do you agree with my choice? or should i go for other?

c box. I wont have any money coming in myself and i am only taking about £600 with me for the wedding fund. So am i suposed to put all zeros? do i declare the £600 as unearned which is only one off so it wont be coming in regularly? Was gunna say i earn and unearns squat

then the financial support evidence. do you think Vitos letter of intent for the interview would suffice?

Oh and to finish off because on the second page it asks if your address and name has changed please provide us with your new details, so i assume that means the first page name and address will be my current name and address that they allready have?

Sorry for all the stupid questions but if someone has filled this out before would be geatful for yours input.
