Filling out the DS-230

Hello,

I have a couple of questions about filling out the DS-230. My husband is the beneficiary and I am the petitioner.

Question #14: Name of Spouse (Maiden or Family Name)

Should I write my maiden name or my married name?

My married name is my legal last name but my marriage certificate contains my maiden name...I am totally confused about this one

Question #6 Place of Birth (City or Town), (Province), (Country)

Bolivia has Departments equivalent to States in the U.S., then there are Provinces, then Cities/Towns.

I don't know if he should write the Department or the Province in the space for Province???

I have another question about the DS-230 actually.

What do I need to send in if my country does standard processing?

I hear different things... some say marriage certificate and all that but that's only if it's not standard processing right? I just need to bring all those documents when I go to the embassy for the interview as I understand it.

I know I have to send a copy of the biographical page in my passport, but do I also need to send a passport size photo?

Anybody? :luv:

Read the LingChe NVC Shortcut Guide. It lists all the docs required for the IV packet. The link is around somewhere.

You need your birth certificate, marriage certificate, four 2x2 passport photos on white background, police clearances, etc.
