You take the photo in with the interview. It has to be a US sized picture; the medical can be a UK sized one. Do a search on the forum; I think there's a website online you can order them from.

Or just look 2 or 3 posts up from this one :)

hi all... quick question..

i sent the checklist in yesterday after setting up my medical date.. however I got confused since it says on the form not to send any documents so this is all I sent in. I take it from what I have been reading I should have included all of the DS forms with this??

Yes you should send in the DS forms, but not any supporting documentation like birth certificates, etc. It is ok if the checklist and the DS forms don't arrive together.

It means not to send in documents such as the police certificate, proof or relationship, birth certificates, etc.

The DS forms ARE REQUIRED to be processed prior to your interview date. The consulate must have the DS forms and the checklist so that you even qualify for an interview date.

I know the wording can be confusing sometimes, but they mean the documents you take to the interview, they are not talking about the DS forms. Must send in DS forms + checklist if you want an interview!

If you send in the checklist without the DS forms, you are partially done with the prereqs for the interview, they will hold on to your checklist until they get the DS forms to begin your processing for an interview date.

Same goes with people who may send in their DS forms without the checklist, they will hold on to the DS forms and wait for the checklist to give you an interview date.

You need DS forms AND checklist for getting an interview date, regardless if you send the forms first and the checklist later, the consulate must be in possession of both sets of documents prior to issuing you an interview date

can anyone give me any advice on finding vaccination records in the UK?? my fiance has asked his GP who says they have nothing, he thought about phoning his old school district, etc.. he has had the normal required vaccinations, he just cannot figure out how to get the records.. and IF he cannot get the records, does he HAVE to get all the jabs at the medical interview? or can he get them when he gets here in the states? or at his GP? if we can avoid having him do all the jabs over again, that would be great! any advice?

You can pay your doctor for your medical history, that should have all his vaccinations on??

I'm having the same problem. My doctor has nothing on record only a flu jab. My dr told me to call child health services as any jabs that you had at school will be recorded with them. However my authority only keeps records from the last 19 years and therefore have nothing on me. I rang the medical drs and they said it should still be ok and that at my medical they will discuss and determine if I have to have the shots again. They also said that if I do need them I can go back to my own dr for them and my dr will then have to fax thru to them written evidence of having had them.

I hope this helps but definitely call the medical drs if you have concerns, they put my mind at rest.

John got his medical and only had to get two shots to complete the requirements for the visa. He said he got 1 that covered tetanus, polio, dpt all in one and the flue jab. He also said.. my doc had a record but wasnt up to date now iv had those they gave me a record at the med said i was up to date now :)

My fiance just went to his GP and they didn't charge him anything for the vaccinations. From what I have read if he cannot locate his records, he will only be required to get 2. The MMR and tetnus.

I called DOS this morning, still could not confirm that London has received the box that DHL shows delivered on 10/19. She told me that it could take them as long as a month. She also told me that sometimes the embassies get the boxes and then don't open for awhile.

I guess they might take a bit since the box the day before was close to 50 pounds? It has been a week and I just really wish they would hurry up. Also, more postal strikes will happen this week, especially in London.

Are there any people lurking that have had this same issue with London?

Jewel, I was told the same thing by DOS. I was also told that our file was sent to the embassy on a date that it wasn't. I tried tracking it through DHL but there were so many packages going to the embassy there was no way to know which one ours was in. They will tell you that but I would just keep calling. I called every other day and finally one day they said his packet 3 had been sent to my fiance. I totally understand how you feel. We sent them the packet 3, 2 weeks ago minus the checklist (that hasnt gotten there yet,mailed over a week ago) and they still don't have it in the system. They must be really busy right now is the only thing I can think. I want them to hurry up too...this part of the waiting to me has been the toughest...

We were delivered to London that same day, possibly in the same package. And on Friday when I called I was told we were in the system. But it was one of those people you talk to that sound like they may have wandered in off the street and answered the phone, I was wholly unsure that she knew what she was talking about. :lol:

So, I called again this morning to double check, because Neil wants to book his medical, and I spoke to another woman who said we were electronically in the system, but they didn't have the hard copy of our case yet. Ummm...not sure how that can even happen. But, they're running long enough with medicals right now, I'm hoping it'll be all received and entered into the system by the time he goes if we're "electronically" there. :wacko:

Just thought I'd let you know. Did you ship out from DHL on the 16th? Because if so we're probably in that same package.

I want them to hurry up too...this part of the waiting to me has been the toughest...

We mailed the packet 3 minus the checklist 10/12. It took a week to get there. David had his medical on 10/16 and the checklist was mailed on 10/19. The checklist hasn't even made it to the embassy yet...not sure why it would take this long...I checked the tracking number on the Royal Mail website...still not there. I called DOS to see if packet 3 had been received and was told that they will wait until the checklist is received to enter it all into the system at once. I am not even sure if this is true. They do have his medical logged in but nothing else. I can get something mailed to him from here quicker than something gets from one side of London to the other...I just don't get it. At this point, we can either wait or send it again...That is all that I know to do.

I heard the same thing roughsweet! That we are in the system electronically but they don't have the hard copy? What the heck? I'm pretty sure they do, since NVC sent it off over a week ago...

Still, one step in the right direction; last week we weren't "in the system."
