in process of fileing for aos, is it acceptable to send immunization sheet ,that my wife received from her k-1 medical interview?

You will need to take the form to the Civil Surgent in your area. There is another for they have to fill out. Please dont let them railroad you into getting a complete physical if your wife's physical is less then 6 months old.

The answer is yes and no.

When I applied for my own AOS, I sent the Vaccination Worksheet with my application and had no problems.

When I applied for my daughter's (K-2) AOS, I also sent the Vaccination Worksheet and received a letter requesting the Vaccination Supplement. So, I went and had the Supplement done by a Civil Surgeon here in the U.S. At her AOS interview, they did NOT ask me for the Supplement. I believe the letter we received was just a form letter they send to everyone once they received the AOS application. So, I had the Supplement done for nothing. I offered it to them before we left because I figured it may as well be in her file than in my file cabinet at home after I had paid for it. The Supplement cost $35.00 to be prepared, with no vaccinations required. I just faxed her Worksheet into the doctor's office and they prepared the Supplement from the information on the Worksheet.

So, the decision to get the Supplement done is your choice. I just figured it was going to be like the requirement for a fishing licence..... they will only ask you for it if you don't have one!!!!!!
