For the question about my passport:

Place of Issuance:

City:

State/Province:

What should I put? The only thing my passport says under 'issuing' is 'GBR' and I'm confused.

I don't know where exactly passports are issued. I've had this one for almost five years :unsure:

Mine was issued in London cos I went to the Victoria passport office. But if you look on the photo page of your passport at the top it will say 'code' or 'issuing code', under this it should say GBR (Great Britain!!) if it was issued over here.
