Totally clueless on the Vaccination Supplement for AOS

Timeline

My husband (arrived with a K1 Visa) does not have any paperwork from the approved Dr who did his medical exam, only a card from his childhood saying he has had ONE shot. And i think its only for Rubella?

What do i do????? Does he just need shots? What kinda? Can someone help me find the answers we need?

We are ready to adjust to conditional permanent residency.

Timeline

Will need to get the required vaccinations or tested "Titer" tests for the required immunities. You will then get these new records transcribed to an I-693 by a US Civil surgeon.

Start with your family doctor or perhaps the local health department about vaccinations or tests. Call around for the best price.

You only need the correct vaccinations for your age group.

My wife got the required immigrations vaccinations just after her visa interview, before coming to the USA, we used that record when adjusting status.

Or does it have to be from a USCIS Civil Surgeon? I'm a little confused about that.

If we get the shots from the health dept do we still need to take that form to get transcribed/signed by a Civil Surgeon?

I also am a little confused about this.. I have read on USCIS' website that I do not need to get another medical done as mine was within the last year. I also have read that USCIS requires my Immunization records. I have my original from Canada and also one that my US approved doc wrote out for me, however, I have not read anywhere that I need them transcribed to a I-163. Could you shed some light on this please for me! Thank you in advance for the help... Sorry if this question has been answered elsewhere, I haven't found it yet! Thanks again!!!
