Hello

I'm filling out the form G-325A, this is probably a dumb question, but I'm sort of confused. In the blank space where they ask my name, they only ask for one family name, but I actually use Two, My mom's and my Father's last name, So which one should I put in family name?, I thought I would only need to put my Father's name, if so, should I put my mother's where it says "All Other names Used".

use whatever that is on your ID or passport...

Exactly. Whenever you fill out any of these forms from now until the end of time, always list your name exactly as on your passport. Be sure each mention of "name" on every form is the same. Not that it will knock you out of the box or anything, but why risk a time delaying RFE for something so simple?
