OK, I got the details of Dr Arnold from here. So, what does he ask for ? just to fax my DS3025 (K1 visa).

I only have a XEROX copy of that ? what do others have ? and it is not v clear also ?

But, my original question still is - xerox of DS3025 is to be provided for I 693 Part II Item 5. Everybody just has the xerox only ,right ?

OK, got a call back from Dr Arnold and now the cost is $ 30. Still its fine with me, seeing the trouble other folks have to undergo for the same.
