Form i 693 -Dr Arnold

Hi All

I sent the form I 693 and my vaccincation supplement to Dr L E Arnold and i received back a sealed envelope

Do i have to still attach my vaccination supplement along with I 485 for or will the sealed envelope have the vaccincation supplemnt

please suggest
