I-134 Please help!!

Hi, I have read the faq and seen the example form but am still a bit confused about the following things:

Question 3: Name of spouse and children accompanying - should my daughter (aged 11) be listed here?

Question 8: 'The following people are wholly/partially dependant on me...' - should my daughter and I be listed on this? The beneficiary is NOT listed in the example but we will be dependant on my fiance until I start work out there?

Question 10: 'I have submitted a visa application on behalf of the following people....' Should both myself and my daughter be listed or just me?

Question 3: Yes, your daughter should be listed here.

Question 8: No, you and your daugher should not be listed on this. Listed here would be anyone in the US that is currently dependent on your fiance, like any children he has.

Question 10: Both of you should be listed here.

Ok, one other thing, at the bottom of the page it asks you to tick if you intend to make specific contributions to support them? So are you meant to tick this to say you will, bearing in mind in question 8 you say not to list them as a dependant? :wacko:

I left that part of the question unchecked. I think that section is probably for people who bring workers or students over and they have to prove that they will provide room and board. Since its my wife coming over, I'm sure I don't have to indicate how much money I'm going to give her or if I'll provide room and board. ;)

I do have another question for the I-134 though. For the life insurance section, I'm active duty military so I assume SGLI will suffice as life insurance, but what does a "cash surrender value" mean? I'm not sure what to put there. Any one else know what that is? Thanks.
