Question and Clarification regarding I-134 Form

Hello my VJ family!

I am about to accomplish the FORM I-134 which will be sent to my fiancee. There are some things that confuse me. I hope you can hepl me figure these out:

1. In item no. 7 says "I am employed or engage in the business of" - I have indicated my full time job. My question is: do I have to include my part time job? If so the space provided in the form is not sufficient. Can I use extra bond paper and mark it as annex A?

2. Again, item no. 7 says "I have life insurance in the sum of" - Wherein I have stated the amount of my life insurance but there was this question such as "with a cash surrender value of" - Does this has a connection with the value or amount of my life insurance? How will I answer this question?

3. In item no. 11 "I intend to make specific contribution to the support of the person named in item 3" - Obviously, my fiancee will be staying in my apartment permanently. Do I need to answer the question for item no. 11? or should I just put N/A?

The present cash surrender value of a life insurance policy is how much will the insurance company pay you if you cancel and surrender the policy, without dying, now. There should be a table of values included in your policy. Not all policies have cash surrender values; e.g., a term life policy has zero cash surrender value.

i just used the example 134 here on the VJ site. you can clear each field then type in what you need then print it up.i just followed the lead of the person who posted the form.if you are over the 125% many people dont include the other info i.e. savings,life insurance etc. its up to you. the new 134 does not need to be notarized, just signed by you. the big thing they seem to look at is your tax/earning papers.go with the IRS tax transcripts and your W-2's.
