Hi VJ!! So, I had my question answered previously (thanks guys!!) that we don't have to do the medical all over again since my fiance just did it in El Salvador a few weeks ago, but it says that we still need to have the civil surgeon complete the vaccination part:

You will, however, be required to complete Part

1, Information about you, and submit the

vaccination section of Part 2 with your

adjustment of status application. A designated

civil surgeon must complete the vaccination

section and Part 5, the Civil Surgeon's

Certification.

So, we still have to pay for an appointment with the civil surgeon, just so s/he can sign the form that my fiance has all his vaccinations? If he doesn't have the vaccinations already, can we just get them at the health department and then bring the verification to the appointment? Or is it better to have the civil surgeon do the vaccinations?

you shouldn't need another physical but suggest you call around, some love to give them and charge an arm and a leg. The one we found charged us $70 to do the form and give her a tetanus shot.

Get them elsewhere and take a record of them to the DCS.

Many (if not most) Civil Surgeons will try and get you to do a full exam again. They will try to trick you by telling lies about how the USCIS has new rules, or they won't accept just the vaccinations, or that the doctor can get in trouble if they just fill out the vaccinations and don't do a full exam. I've heard or read about all of their lies. Don't believe any of it. They just want to separate you from more of your money.

All you need is to make an appt with a CS and have them transcribe the Vaccination Worksheet (form DS-3025) that your wife was given at her medical in El S. onto an I-693 form.

Show them the instructions if they don't agree, it may help. But know that many will NOT just agree to do the vaccinations section. They will only agree to do another exam, and charge you a lot for it.

Btw, you can get the vaccines you need at your local doctor or health dept for cheap, don't pay crazy CS prices for them.

Call a few different CS in your area to see which ones are reasonable and which ones are cold-blooded crooks.

Many VJers call and mail their DS-3025 to Dr. Arnold in Texas for only a modest fee of about $20.

Timeline

When applying for AOS for my husband and his 2 kids I mailed in a copy of the DS-3025 which for all of them stated vaccination completed.

We did not get the new form.

Our case got transferred to CSC in November. No RFE.

It seems that this is still working when vaccination was completed with the original exam.

What is the DS-3025? We never filled that one out, as far as I know, and my fiance never got a form from the doctor, they just sent everything directly to the embassy.

We're calling around to different CS to see who is the cheapest. Luckily we found a few that said they will fill out the vaccination part for only around $30, so the major cost will be the vaccinations themselves. Strangely, different doctors are saying he needs different vaccines??? This seems like something that should be standardized. I guess we'll go with whoever says he needs the least, hahaha, since they'll be signing the form.

Since we just went through this, and I know it is confusing, i will tell you how it went for us:

We needed Tetnus, MMR, TB Skin test...he already had Hep B but I don't think this was necessary. We called the USCIS doctor we had an appointment with and asked them what he needed. What is needed can be different for everyone. If he had Chicken Pox as a child, he won't need this shot, otherwise he will. His age will make a difference on what shots he needs as well, and his medical background.

If you can find out what you need from the USCIS doctor you are going to see, you can go to a local public health clinic and get the shots done for cheaper, and then go to the USCIS doctor to fill out the forms. You are correct in that you don't need a full medical examination again, because you had this already before entering on your K-1 visa and it was within one year of filing the I-485.

Since we just went through this, and I know it is confusing, i will tell you how it went for us:

Also.....We didn't use the DS-3025....the I-485 requires the I-693 and you will use only 3 pages of it....don't sign section 1 until you are in front of the doctor. They recently published a new version of this form, so make sure you are using the correct one.

Hello everyone,

I just mailed my vaccination supplement back to the USCIS per the RFE given to me by my immigration officer after my interview in St Louis on January 13th.

She didn't keep us very long, she just asked for the employment letter that came from my hubby's work, pictures of us together, she also copied our bank statements and asked a few questions. And then that was it. After we gave her everything and told her the story about how we met, she told me and my Husband that everything else is okay i just need to get the Td shot that I didn't get during my medical exam back home and that I should just go to a CS that serves my area, get the shot and have I-693A form signed and filled out. Soon as they get that, they will send my permanent green card in the mail.

The day after my interview i called the closest CS i could find (he is in Nashville, IL) and nobody knew what in the world I was talking about... The lady thought it was weird that I wasn't required a physical. I made the appointment anyway and when I got there i found out that they didn't take checks or credit cards!!! And that's not all, they charged me almost $200 for a freaking Td shot!!! It was hell being in that doctor's office!!! :angry:

And then i found out when i got home that he didn't fill out the whole vaccination section so i had to call his office again to see if I could come and see him so he can fill the darn form out. They did take me in the next day and got a phone call that same night that i could come pick up the stuff because he has filled it out. He didn't give me a copy this time around so I don't know if he did finish it or not... This is making me all nervous but I mailed it to the USCIS anyway. They got it on Tuesday thru USPS Certified Mail.

Hopefully, everything else will go smoothly. So all of you others out there who are going through hell during this whole process, know that you are not alone...

Since you didn't ask the price ahead of time, they can charge you whatever they want. :bonk:

It was $40 for the Td shot and $160 for the medical (& form). :rofl:

Hopefully, you learned a lesson.

True (we did), but do NOT tell or admit to them that it's for immigration purposes, or they may send you away! We went for tetanus and rubella, and they asked that question first. I had been tipped off to this possibility, and I was prepared. I said, "Eventually, it will be part of that, but immediately, we'll be working with metal, and we also want to make sure that if she gets pregnant, the baby won't have Congenital Rubella Syndrome." Before I'd finished speaking, the nurse began ushering the wife into the examination room, and the administrative lady kindly logged the new shots onto the bottom of the vaccination record from Ecuador.

And now, I can claim that I had my wife shot. Si, man! :)

06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

what about the tb skin test? I will be heading to usa soon and am quite confused over the requirements.

do you just get a CS to fill out the 1-693, (transcribed from the DS-3025?) and with no mention of a tb skin test at all?

or do you still have to get a tb skin test done? and how is that attached to the 1-693 form?

Also, if you do have to get a tb skin test done. what do you say to the health clinic, if you musn't mention it is for immigration purposes?
