If she took your last name, she should file AO with that name as Family name

whichever name she is intending to use in the future - a lot of wives keep their original name

Her legal last name has been changed, and is now your last name.

It only seems reasonable to use her legal name (even though many have chosen to use their maiden name for AoS/GC).

a marriage/certificate does not automatically legally change a woman's last name, it effectively gives her a freebie option to change her name with the different agencies if she so wished

She would use her "current legal name". It's a present tense question.
