Immuniation records from St lukes?

Hello! I'm basically panicking right now. My 4 yr old daughter is supposed to register to a daycare but was asked about a physician's form and I couldn't give any. What I have here is her baby's book that wasn't even updated by St. Luke's after giving her the booster shots before we flew here. And I don't have the Vaccination Documentation Worksheet for my daughter because at that time they gave me a disc attached to this brown envelope and they said its her medical tests that's in there so basically no more x-ray film that's so bulky. Everything was in that disc but was given to the officer in that port of entry. How do I retrieve medical records/immunization records of booster shots from St. Luke's? Do I contact immigration or the st luke's hospital back in the Philippines? anyone has encountered this problem? Please help. I'm supposed to start work next week but cant if my daughter isn't accepted in the daycare. thanks
