Daycare issues about a certain physician's form for daughter...

Hello! Im not sure if this is the right place to post this problem but I just hope I get some substantial answers. We got here in the US (via CR1 and CR2 visas) 3 months ago and I have a daughter whom we plan to put into daycare so I can start a new job. The thing is the daycare director is asking for a physician's form stating she has completed vaccination and she's completely healthy. We all know that before we can come to this country we need to have completed the vaccination first and go through a lot of tests. And I have no proof of that now because all my daughter's important documents were given at the port of entry. And also my daughter doesnt have health insurance as of the momet so going to a pediatrician here is not an option. Right now we can't afford it. We are still suffering the backlog of immigration costs and all. Where can we get a copy of a physician's form of immigration? How? Please help. thanks.

I jsut wanted to say that if you google "health clinics" in your area, you will most likely find one that works on a "sliding scale", they will use your income to determine your copay for a visit. Most also offer prescriptions fillled for minimal costs as well

If you were lucky and got an approval and reunion with your loved one rather quickly; Please refrain from telling people who waited 6+ months just to get out of a service center to "chill out" or to "stop whining" It's insensitive,and unecessary. Once you walk a mile in their shoes you will understand and be heard.

Did you daughter have a physician in the Philipines you could contact for a "health letter"?

When I immigrated with my twins, I had our family doctor in Ireland write a letter on his headed notepaper with a list of all vaccinations given (including type and batch number), and record of all other visits we had.

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.

yes our private doctor could give a record but the boosters were all given by physician by US embassy and i dont know how to contact them. the sad thing is when my daughter had all those booster shots they didnt even bother updating her baby book.

What you need to do is getting the information of the shots received. Is there any way to contact the doctor who gave the shots?

Do you have any idea what shots she got?

You should have been handed a vaccination record, but nothing you can do about now.

Go to the Health Department of your county. Get an appointment for a 'physical'. That's what the daycare wants. My children also had their medical done overseas, but for elementary school and daycare a 'physical' was required. They are using a sliding scale regarding payment, so if you don't have a lot of income, you don't have to pay a lot.

They also will discuss the vaccines with you. Maybe they have a solution.

My step Kids who we enrolled in public school needed an updated TB test so when I took them to the doctor he went ahead and passed all the information from the embassy doctor form and the kids doctors over the years onto the regular paoer needed here in the US. Do you have health insurance? If so just make an appointment at your childs doctor and have the information passed onto the paper the day care needs.

If there's one thing that you should understand about the US, is that we put children above all else, they are our future. No matter if they are immigrants, sponsored, illegal, doesn't matter. there are ALWAYS FREE medical benefits for your children EVERYWHERE.

So, if you cannot locate the records, do not despair. Where are you located? You have to find the local free clinic (Usually some kind of program at your local hospital). Fill out a few forms, and your child will get the vacinations and medical care that he/she needs.

As far as the child care center is concerned, they are not worried about booster shots, they are more concerned about communicable diseases that can be highly contagious.
