Does anyone have the paper required for the Dr visit?? We would like to have it ready so that when the time comes, we can see the dr before our first interview and have all things ready...

USC Received Tunisian Residance Card December 1, 2008

DCF CR1 - Tunisia Consulate

Applied I-130 on May 5, 2009

Received Packet 3 - May 30, 2009

Received Packet 4 - June 8, 2009

Final Interview - June 11, 2009

Visa Approved - June 11, 2009

Visa Printed - June 12, 2009

ENTERED USA - JUNE 24, 2009 !!!

Green card received - July 16, 2009

Social Security number - August 18, 2009

Timeline

Foreign panel physicians authorized by the consulate to perform immigration medicals should have the required forms.... otherwise it is forms DS-2053 & DS-3025 (maybe others as well)
