Why do they requesting all of these information?

Is it ok to not provide additional information on application for passport such as telephone (home, cell, work), employer, e-mail address, emergency contact, travel plans…

Have to take your application into an agent when doing it locally, like the post office or city clerk, DOS has a list in their site, have to take an oath that all information is correct and will have your form carefully looked over, but they cannot verify if your certificate of citizenship is valid, the original has to be sent in. If you really want a US passport, will comply with all of the requests.

Sure, but I do not have to answer to questions that are considered optional… I do not think e-mail, telephone, employer, travel plans are necessary in processing passport app.

if they are OPTIONAL for a passport application, you do not have to do, thus they are optional! It is very clearly pointed out on the online passport application on what is "optional" I did not add any optional info and I just got my passport in the mail this week. 9 days from mailing it out, to receiving it!

Canadians Visiting the USA while undergoing the visa process, my free advice:

1) Always tell the TRUTH. never lie to the POE officer

2) Be confident in ur replies

3) keep ur response short and to the point, don't tell ur life story!!

4) look the POE officer in the eye when speaking to them. They are looking for people lieing and have been trained to find them!

5) Pack light! No job resumes with you

6) Bring ties to Canada (letter from employer when ur expected back at work, lease, etc etc)

7) Always be polite, being rude isn't going to get ya anywhere, and could make things worse!!

8) Have a plan in case u do get denied (be polite) It wont harm ur visa application if ur denied, that is if ur polite and didn't lie! Refer to #1

Yes it is optional, but a little short sighted not to provide that information. There are pieces of information that are submitted with your passport that allow the US authorities to contact you or your family if need be. For example:

- You are abroad and involved in a traffic accident, become ill, taken hostage, killed, go missing, kidnapped, get arrested etc. The local police can contact the US embassy and give the embassy your passport, and the Department of State can look at your email, phone, employer, home address etc. and get in contact with your family for you if you need help.

- You are abroad and the Department of State issues a warning for that country (e.g. imminent civil unrest, natural disaster, disease, terrorist threats etc.) and needs to contact US citizens in that country to warn them. The Department of State can look at your email, phone, employer, home address on file etc. and get in contact with you and your family to get critical information to you.

Sure, you don't have to provide that information, but there's really no good reason not to, particularly when the purpose of giving that information is for your personal and sole benefit. It's not as if the Department of State will use it to send junk mail to your house, or spam your email address with wiener enlargement messages.

Your point makes sense too. But in today’s world where we are moving and changing our phones, emails, employers etc so often. For example I have changed apt a few times along with the phone number.

They think it's important. You don't. It's optional to give the information, so don't give it. It's not a threadworthy topic.

Sure, the information changes once in a while, like your address when you move. Although in today's world of email and cell phones, there's actually little reason for most of your contact info to change each time you move.

I'm intrigued about one thing - you're making such a fuss about this matter that it makes me wonder why you are so desperate to hide that information from the government?

I'm beginning to wonder with all these questions is he/she making their own passport? Marco? By any chance have you been featured on America's Most Wanted?
