Positive skin test means you were exposed to tb but it does not mean you have a tb. If you had a BCG vaccine before that would most likely make the mantoux test/skin test positive too (not true to all cases). Chest-xray would not even tell that you have tb too. The gold standard test would be the culture test. When we visited the tb clinic here in DC the nurse insisted that I will have a skin test and I & my husband hesitated for her to do that bec I have a Chest Xray and sputum result with me. However, when we met the physician we told him that we have the xray and sputum result and he was like, what are you here for? Then. he gave us the clearance. You can still file the AOS package as long as the civil surgeon will sign your I-693 with negative tb. That's my two cents.

Having a positive skin test does not mean that you have an active TB infection. A positive skin test coupled with negative X-ray could mean (1) You were exposed to TB germs at some point in your life but don’t have active germs in your body or (2) You had a TB vaccine (BCG).

Most of the civil surgeons would categorize your situation (positive skin, negative x-ray) as latent TB infection (LTBI). In most of LTBI cases, civil surgeon would not recommend you for any further treatment and would sign the I-693 right away. But if he has some reason to believe that you are carrying active infection, he will ask you to visit the local Health Department and get treated.

Your problem is that you can’t go through an X-ray to override your positive skin test. My guess is the civil surgeon won’t sign I-693 without a negative X-ray, so ask him if he will accept your recent X-ray from Philippines.
