No Ds-3025?

I recently brought my fiance' to the US on the K1 visa. I am now filing the AOS forms. She had a medical exam in CHINA and they gave her a sealed envelope that she gave to the guard at the POA, but she never received a ds-3025. What they gave her in CHINA to take with her was two small passport sized books. One is yellow and says "international certificate of vaccination or prophylaxis". The other is brown and say "certificate of health Examination". I know we are supposed to turn in a vaccination supplement, I-693 or a least a ds-3025 with the package. What do we do. Will a civil surgeon except these to fill out form I-693?

Yes, but you might have to shop around.

after rereading the faq at the top of the forum it sounds like I might not have to send in any medical info. still confused. can any one clarify?

the I-485 instructions say; Page 4 part 8.

" Fiancé(e)s.

If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your adjustment-of-status application."

the faq says;

"If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693."

She did not receive Form DS-3025.

the I-485 instructions are confusing 1st they say; "you only need to submit a vaccination supplement". Then the next sentence says; "You may include the vaccination supplement". What the heck does that mean?

hi, well, i am not 100% sure whether you need it or not, however, for safety sake, I would recommend you to bring that "yellow book" to the civil surgeon, since that yellow book should have listed her medical records and the civil surgeon would know how to work on her medical examination. Just to make your application go smooth, I would suggest to do that medical examination from the civil surgeon. GOOD LUCK!
