Great news today in the mail box. We got an appointment letter for Feburary 25th, So now here is my question as always lol. We came to the US with a K1 visa and our AOS interview letter says that we need a completed medical examination form (I-693) and vaccination supplement in a sealed envelope(unless already sumbmitted) ! My question is was this done in Juarez before we came was this in the sealed envelope they gave us or is this something we need to do before we go to the interview ? Any insight or thoughts would be helpful ! Thanks and God bless Chuck and Teresa

You should have got a DS 3025 vaccination record. Take that to a USCIS designated civil surgeon [DCS] and have him fill out the I-693. You ONLY need the vaccination part. No need a full medical.

Most DCS will try and hit you up for the full medical. Shop around. Try Dr. Arnold also. I don't know if she is still doing the I-693!

She did/does it by fax and phone. Price used to be $20.

Thanks for the reply ! My wife did not get the DS 3025 vaccination record. So I am not sure how to get the form. I also will try to Dr. Arnold She does have her vaccination record do you think this will be enough as we have entered the US on the K1 visa. Thanks Chuck and Teresa

Double check for the DS 3025 in her papers. Form says DS 3025 in SMALL print at the bottom. DCS will determine what vaccines are necessary if she doesn't have any records.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship
