Removal of the Conditional Status ( approved Sept. 9, 2011)

sis, DS 157

with the validation at the back.

BDO will give you a stamped DS 157. You have to fill it by hand and make a copy.

Try taking a filled out DS 157 and they "may" stamp it. If not you can use it for your copy.

It's DS-157,sis...

I dunno if they'll take the form you already filled out. They would usually gave you a blank DS-157 form already validated with the visa payment. So you have no choice either to type write it or have it handwritten.

usually they gave the blank form. so u can type or hand written it.

i know Radz went to bank in Davao with DS 157 already filled up, they would not let her use that one and had her fill out another one which they validated. I have read some that take the form and they accept already filled out some not. But it didnt really matter she had the filled out one with her and just transfered the infromation over to the form they gave her printed neatly. Good Luck

Thank you guys for all the replies. I prepared one DS 157 online, if that`s the case I may just use it as reference when I fill up in the bank, and hope too that other BDO branch especially in Makati accept filled up form of DS 157, its better and more presentable if its already filled up from online, but in case I will comply what the bank says.God bless and Happy New Year to all!
