I need to prove:

Please provide a sworn affidavits (must be signed in front of notary /solicitor )sworn to or affirmed by third parties having personal knowledge of the bona fide of your relationship with the beneficiary. (Each affidavit must contain the full name and address, date and place of birth place making the affidavit; if any, to the petitioner ,beneficiary and complete information and details explaining how the person acquired his or her knowledge . the affiant may be required to testify before an immigration officer about the information contained in the affidavit.

Does anyone have a template they have used that I could customize for my case?

Any help would be greatly appreciated.

It's a letter. Use a letter template if you must. Be advised affidavits do not prove bona fides, no matter what is "sworn to" in them. Generally for newlyweds filing I-130 petitions, the "any other evidence" will be more than sufficient. Examples would be pictures and other evidence of time spent together.
