Cud someone please help me with the following questions:

I 765 Questions:

1) I got temperoray EAd at JFK.In the Questions 11 ,have u ever applied for employment authorization from USCIS,should i say Yes or No?

This is the first time i am applying for EAD but i already got a temporary one at the airport,so m comfused

2)JFK was the port of entry for me but then i came to Washington DC..So what should i write for Place of Last Entry in to US

3) I am applying for EAD,Advance PArole and AOS together.So what should be answer to Question 16,eligibility categories.

Should it be a6 or should it be c9.

If i write c9 then they ask for evidence to prove that my I 485 is pending,

3) I am applying for EAD,Advance PArole and AOS together.So what should be answer to Question 16,eligibility categories.

Should it be a6 or should it be c9. You're K1 so it's "C9

If i write c9 then they ask for evidence to prove that my I 485 is pending, They wont if you're filing everything together (at the same time).
