Hello,

My wife has her biometics appoitment this week. I'm wondering what this entails. Specifically she's not all that strong at filling out forms (this is very new to her). I'm not going to be present due to work so I'm wondering what type of information they are going to ask. Will they ask about previous addresses, travel outside of her own country, father and mother's name and birthdate, brothers and sisters, children?

I don't remember there being any forms, but they did ask a few questions verbally. Then, they just do the fingerprinting and you're on your way.

There was a one page form that asked the following. It was pretty basic. I only remember what I do because we had to do it twice (for EAD & AOS) and I filled out the forms for my husband. I could be missing something....sorry. She will need to bring the NOA letter and a form of identification with her. My husband has a State ID -- they wanted that -- we also brought his Passport just in case.

Name -- Address -- Phone Number

Social Security Number

Date of Birth

Parents Names

Height -- Weight -- Hair Color -- Eye Color

Country of Birth

Alien Registration Number (A#)

Receipt Number (MSC#)

....and you had to circle the "number" at the bottom of the page for what you were there for
