I am about to submit my AOS package, and just realized the I-693 medical form that my civil surgeon gave me never required a signature from me. It is sealed, i can not open it to check. but i think the top part needs my signature... does anyone know what the procedures should be? thanks!

no sig required other than the CS... do not open the sealed envelope... submit as is

I think this is incorrect. I had to sign it, before the exam. The question is if USCIS accepts it without a signature.

I would call the civil surgeon and demand them to have me sign it. After all, you are paying them to do a correct job.

I certify under penalty of perjury under United States law that I am the person who is identified in Part 1 of this Form I-693, Report of Medical

Examination and Vaccination Record, and that the information in Part 1 of this form is true to the best of my knowledge. I understand the purpose of

this medical exam, and I authorize the required tests and procedures to be completed. If it is determined that I willfully misrepresented a material fact

or provided false/altered information or documents with regard to my medical exam, I understand that any immigration benefit I derived from this

medical exam may be revoked, that I may be removed from the United States, and that I may be subject to civil or criminal penalties.

Signature - Do not sign or date this form until instructed to do so by the civil surgeon Date (mm/dd/yyyy)

My husband signed his before it was sealed by the CS, too. I would definitely call them and request that they re-seal it with your signature.

My son did not sign... If you are worried then here is an idea, copy the verbage on to a separate sheet of paper and sign it and adhere it to the envelope...
