I a Health Depatment oke?

I called yesterday a Civil Suregeon in MS..I told them that all I need is the vaccination supplement,since I've done my medical examintaion overseas within one year.They gave a number for a Healh Departament.Tomorrow I go there...but I want to amke sure they are certified to do that,

I've seen on form I-693 there is a part 6 and aprt 4 that can be completed by a Health Department..Do I still need a civil surgeon signiture at part 5 in this case?

Anybody did that before??

Health department can administer vaccinations but are not authorized to prepare and sign the I-693, only a designated civil surgeon can do that

Yes the health department is fine. I actually took my step daughter down to our local Vaccination Clinic and they did all her shots for free since she was under a certain age. Then I just took that paper work back to the Civil Surgeon and he transferred it.

I actually don't need any shot,just the transcript but for that all the civil surgeons that I called ask for my HIV tests and all stuff that I"ve done 2 months ago for the K1 inetrview...and of course I don't have the results!As everybody knows they were in the yellow envelope..sealed...God@!
