I've just called 5-6 civil surgeons!!!and they all tell me that I need to do the blood tests again or they need a copy of the results!!!

I was not given a copy.They were all in the yellow envelope.iI suppose.All I have is the vaccination list and they don't want to transfer the results on form I-693 A without the entire medical examination!!!An I told them I did them 2 months ago,in order to get my visa!!!Most of them were so unpolite ane they told me not to call them again...and they wanted to charge me around $3oo!!!!!!!!!!!

What is that?smbd help me???

I would send to USCIS just a copy of my vacination list from Ro,but....I've read in the instruction that ts advicible to send Form I-693 because the list can be lost...and they were denied cases because of that!!!

It's crazy!

I'm located in LA.Anybody in my situation?

Many C.S. dont bother reading the instructions included with form I-693, (There is NO I-693A anymore) the instructions with form I-693 explicitly says that a FULL medical is NOT needed for K-Visa holders if they had a medical for their visa less than a year before filing of AOS.

A simple solution is to do this in the mail with the doctor in TX.

AShe said that I need to fill out the first part and they need to fill out page 3 and 4.

What about part5/6?I suppose that this is a health department so they should fill out part 6,right?and sign and put a stamp...

Should be any pb if it;s not a civil surgeon?

Do you know is it's legal to change my middle anme also?To change it into my maiden fam name?I wanna do that in all my AOS papers,or it;s better to keep my middle name as it was before marrige?..Caz our marrige certificate it doesn't say explicitely my new name..my husband has his name and I have my name...but I've read on the forum that I can use from now on his fam name...I didn't find anything about my middle name...

My Yu had no middle name so she took her family name as middle name and took my family name as her new family name. This is fine.

I went to a CS in Santa Ana and he just charged us for doing the form. He did want an x-ray but I had the one from the embassy and he accepted that. Being a civil surgeon, in some eyes, gives you the right to rob people :angry:

If you don't mind driving to Santa Ana try this guy:

Make sure you let them know it is only the supplemental vaccination form that you need filled out. You will need that DS3025 (I think that is what it is called) from the embassy. Also ask for a copy of the supplemental before he seals it.

If this guy wants to charge you more expand your search. You should find someone reasonable in teh greater L.A. area that won't charge an arm and leg.

But I already have a middle name:)so..shoul I keep it..I've read in the SSN politics that is not leggal to change it in my maiden fam name?Am I wrong?

Thank you....I can't go in CA.I'm located in LA...but I will go on Friday on a Health CEnter.A civil surgeon send me there.I've seen on I-93 that there's a field where can be fiiled out by a Health Center reffered by a civil Surgeon.In this case I think I need the civil surgen stam also...I have no idea.It's just my supposition.
