Hello to All:

Graham and I are waiting for pkt 4 to arrive, but we'd like to know if we could book his medical before that arrives in the post...

Are we able to book that with his case #?

And what are the forms they send in pkt 4 pertaining to the medical?

Is it just the vacc. sheet and a ds-156 that I need to download ???

Thank You !

Rose

PS-I do appreciate all of your advice lately...you ALL have been so helpful to us. : )

Rose,

I booked my husband for his medical way before I got packet 4, I just used his case number and that was it...

As for the forms in packet 4 I am not sure, as they had received the medical results before sending the packet to us, and I reckon that they just didn't want to bother.

I do reccommend that if he needs any vacc. done, you do it with your GP and ask for proof of this... It will be far cheaper that have'em done at Knightbridge...

Hope it helps!

Thank you Maria...

Are there any forms I should download to bring along?

And, is the UK "vaccine booklet" good enough proof?

Thanks !

Hey, sounds like a dumb question but how do you know my name?

As for the forms, you dont need to download anything to bring with you for your medical. When you book it they will send you a checklist of what you need to bring with you, ie, passport, 2 photos, etc.

Your little UK vaccine booklet is good enough proof of what vaccinations you have had, I had the same. Keep that booklet safe as I also had to use it for doing my AOS application when I got to the US.

Check list cmoes with packet 3 - its called an IV15 and I belive someone has put it on a thread in here, UK forum, somewhere for download ;)

I put a scan of page 2 of the IV15 on the same thread if that helps you locate it.
