Hi, my wife is not present in USA. Seems w7 form needs to be filled

I have two questions

1. in 1040, what should I put in spouse's SS#? should i leave it empty or put ITIN TO BE REQUESTED?

2. when filling out form W-7, can my wife put in my mailing address (in USA although she isnt here)? I want her ITIN number mailed to me as it takes a long time t o get mail in her country(and its not reliable also)

johnsmith,

1. I would leave it empty

2. Have you looked at W-7 yet? The answer should be obvious.

2. You can put your address on #2 and her address on #3. (I chose to use my address here in Canada as the mailing address).

By the way, depending on where your wife lives, you may need to get a notarized copy (photocopy) of the biographical page of her passport. In Canada, we are required to go to a US consulate because they wanted a US notary to do this. That's what I did. They also gave me the W-7 and made me fill it out in front of them. Then, they attached the W-7 to the notarized copy of the passport. Just check things out and see what they need. Also, the W-7 has very detailed instructions on how to apply for the ITIN.

You leave it empty

You put your address as mailing address

You will need a notorized copy of your wife's passport. I just sent the original myself. The ITIN is to be sent in with your 1040.

#1 Leave it empty as Yodrak said. I put "ITIN requested" & IRS whited that out.

Also, please read the instructions carefully of the W7 as she can use other means of ID besides notarized passport. (I certainly wouldn't take the chance of her mailing her original passport with the mail as you say it is in her country) If you decide to use a notarized copy of her passport, some countries are part of The Hague Convention & you can use local notaries and it will be accepted by IRS, that's what we did here in New Zealand Google for that info.
