What documents did you submit with your application?

We sent:

- Copy of front and back of my Permanent Resident Card and my daughter’s Permanent Resident Card

- Copies of my Driver’s License and my husband’s Driver’s License showing same address

- Copy of our Social Security Cards

- Copy of our joint Savings Account statements (we sent 8 statements)

- Copy of our Joint 2006 and 2007 Tax Returns

- Copies of 2006-2007-2008 utilities bills - joint accounts

- Copy of joint ownership on our house

- Copy of joint homeowner’s insurance policy

- Copy of my daughter’s Progress Report Cards from her School - my husband is contact person

- Copy of Insurance cards showing me and my daughter on my husband’s health plan.

- Copy of my husband’s Union Records - I am the beneficiary

- Copy of joint ownership on our cars and boat

- Copy of vet bill for our pets showing our names and our residence address

- Mail addressed to both of us

- Pictures with 3 of us showing that we visited places and attended events together.

We moved and lived for few months in another state so we sent:

- Copy of the rental agreement for the period we lived there

- Copies of utility bills covering the period we lived there

- Copy of some pay stubs showing that we worked there and our residence address

We sent to USCIS - I-865 and filed online for me and my daughter change of address when we moved and again when we moved back.
