MY FIANCE JUST RECIEVED PACKET 3. MY QUESTION IS HE GOT A FORM WHICH SAYS OBTAIN FOLLOWING DOCUMENTS ON CHECKLIST WHICH PERTAIN TO YOU., SO DO I SEND THAT PAGE OR JUST LAST PAGE :APPLICANTS STATEMENT

HE DOD NOT GET DS 156. ONLY DS 230 PART1

Most consulates want just the DS-230 part 1 and the check list sent back to them.

DS-156 tends to be in P4 and you can do the form on-line this will produce a barcode that contains all information entered into the form.
