When NOT to use form I-102

Timeline

My wife was issued form I-94 when she entered the US in Chicago. She washed it the other day and even thought he form is legible the stamp was washed away. On the replacement form I-102 it says not to use this form "to request an action on Form I-94 issued by US Customs & Border Patrol. If you are seeking a new form I-94 based on form you should contact the nearest CBP office or port of entry" Has anyone been through this?

Not a problem, as long as the pre-printed I-94 number is readable, the record should be able to be looked up by DHS, which oversees USCIS and CBP.

You may want to contact and visit nearest CBP office to see about getting the card re-stamped.

What does she need the I-94 for?

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship
