vacc. doc. worksheet says not completed

I had a friend here who arrived in K-1 also and her vacc. worksheet says it's not complete but she received all the vaccines that's appropriate for her age only and no need for more vaccine in US. Does she need to transcribe it to a civil surgeon or she can submit the DS-3025 only. Thanks for those who can reply soon.

Timeline:

2-02-08: Honey arrived in Manila

2-04-08: travel down to my hometown, Zamboanga

2-07-08: Our Engagement day

2-27-08: 1129F sent to CSC

2-29-08: NOA1

3-02-08: first touch

7-11-08: 2nd touch- APPROVED!!!!

7-14-08: received email from CRIS that our petition has been approved July 11'08

7-16-08: received hardcopy of NOA2 in our mailbox

7-23-08 hard copy from NVC

7-28-08 paid delbros for DV

8-19-08 medical (LORD, pls. keep me physically fit)

8-27-08 Interview, got pink slip, praise God

9-05-08 received my visa, had my CFO the same day

9-06-08 flight to US, POE LAX

11-08-08 wedding day

11-12-08 ssn application under my husband's name

02-26-09 sent AOS packet to Chicago Lockbox thru USPS Express

02-27-09 recieved & signed by V.Bustamante

03-05-09 cashed check

03-09-09 AOS, EAD, AP received

03-12-09 Biometrics appointment notice recieved

03-28-09 sked for boimetrics, 2pm,done

04-13-09 received letter for initial interview sked 05-13-09

04-23-09 AP received

04-27-09 EAD received

05-13-09 passed GC interview, my status online says GC production ordered

05-19-09 Welcome letter received

06-24-09 Green Card receive

02-13-11 Lifting of Condition

02-09-11 ROC package sent

02-18-11 recieved NOA, waiting game

As for my experienced I managed to submit only the copy of my DS 3025 but medical happened before June 1, I never went to civil surgeon nor transcribed my vaccine, below is my timeline I took only 3 months to approved my GC.

wow, impressive!!!!! anyway thanks for a quick reply but i guessed her medical exam was done last 2 months ago so. she thought of submitting it without being transcribe by a cs.

April 24, 2014 - In line for interview

August 25, 2014-Scheduled for Interview

September 30, 2014 - Interview and Oath-Taking :joy:

I guess case to case basis, I did what sunlyk did and got my GC in 3 mos, if your friend is willing to do this advice , he/she can submit what she/he medical have to her/his AOS documents but if he/she in doubt contact Dr. Arnold. There were appicants here in vj consulted different Civil Surgeons and paid different amount, some were $50 and some paid more than $100. they got RFE and some were not. so I assumed If depends on the personnel of USCIS intrepretation. this is just an idea on how the process works..

07-20-07- first visit to phils

07-26-07-engagement

07-28-07- back to US

09-14-07- submitted I-129F

09-25-07- NOA1

10-25-07 -NOA2

11-01-07- My fiance got a letter from NVC that within 2 weeks Manila Consulate will send my Packet

11-20-07- Date of my Packet, actually the real one was earlier but it was sent somewhere, good thing my Fiance arrived to spent Thanksgiving day in Phils and we checked the status in Embassy.

11-23-07- Medical though the exact date was Nov. 29..

12-06-07- Interview

12-16-07- Visa in hand

01-16-98- Attended CFO

01-18-08- Arrived in USA , POE-Guam

03-16-08- Wedding

AOS

04-17-08- Submitted the AOS

04-24-08- NOA

05-13-08- Biometrics

05-27-08- Transferred to California

06-14-08- EAD card received

07-21-08- Date of AOS approval but got the letter July 24

No Interview, No RFE and Additional Vaccination( just used the SLEC medical docs)

07-22-08- Date of Permanent REsident CARd , got it July 29

07-31-08- apply for SSN

08-05-08-SSN delivery

01-12-09- First day of work(Thanks to Marielle and Mario)
