Hello Everyone!

I was going to have my love get started on some of the shots required for the medical but her doctor told her that a form issued by the embassy was required so he could record the shot dates and other information.

I have not heard of a form issued by the embassy... :wacko:

Is this downloadable or is the doctor mistaken?

I assume you are applying for AOS, am I right? If I am, your wife should have her vaccination record with her. It's given along with her X-ray film from St. Luke's. The shots record will serve as reference for the CS to see which other shots she needs to get as required and transcribe all of the shots she got on the form to be submitted to USCIS for your AOS application.

I assume you are applying for AOS, am I right? If I am, your wife should have her vaccination record with her. It's given along with her X-ray film from St. Luke's. The shots record will serve as reference for the CS to see which other shots she needs to get as required and transcribe all of the shots she got on the form to be submitted to USCIS for your AOS application.

Look at their timeline first, it is too soon for AOS.

The forms tend to be at St Luke's. The immigrations doctors know which vaccinations are required by age group.

I had my wife do the MMR series and HB series before her medical in the Philippines. She just took the record of such to her St. Lukes medical and didn't have any problems.

Be sure she gets the DS 3025 vaccination records from St. Lukes when she gets her XRay record [CD?] on the second day.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

I suggest that she gets her shots in St. Luke's because it is included in the medical fee. That will save you bucks getting vaccination from another doctor.

Hi, Thanks for all the replies!

I understand that the doctors at St. Lukes give the shots and that its included in the cost there.

However...

Everything I read here at VJ says the doctors will only give a few.

anti cervical cancer,MMR, HPV and varicella to name a couple...

The list of required shots is about 20 long...

No one can get all of those at one time. Not and live anyway... :)

I wanted to get her started on some of the others on the list but was confused about this form the doctor insisted we needed to get from the embassy...
