when shall i book my medical

hi

i have my interview on 5 january 2009

i havent recieved the letter yet i phoned uscis and found out as nvc wouldnt tell me

*my question is when is it best to book my medical

*i phoned the london knightbrigde medical center and booked a medical for fridaay 5 december2009 i told them i dont have the letter yet and they said its fine as long as

the visa interview is ready

For appointments, please call 020 7486 7822 (Monday to Friday, 9am to 5pm).

Please have the following information ready when you call us:

Appointment dates you prefer (this should be at least 4 days before your interview date)

Full name and date of birth of all applicants requiring a medical exam

Case (LND) number

Interview date (if already scheduled)

Credit card details (to secure your reservation only- you will be charged £80 if you cancel your appointment within 3 days of your appointment date)

Contact phone number or email

WHAT TO BRING TO YOUR MEDICAL EXAMINATION

Appointment letter from the United States Embassy

Passport

Passport-sized photograph (one for each applicant, including babies)

Medical questionnaire, form IV18a (one for each applicant, including babies)

Spectacles or contact lenses if worn

Vaccination records , with original certification

Children's 'Red Book' health record

Medical reports of any past or current illnesses including psychiatric

Let me know if you go through with your medical without the appointment letter and medical questionnaire form IV18a.

I looked and I can't find that form anywhere. I too was gonna download so my husband could get the medical over with now. But it doesn't look like thats an option. still waiting for information from the embassy
