My husband came to the US with a K1 visa. We know if his medical exam was done within this year, we may submit his vaccination supplement instead of the entire medical report (which we don’t have because we didn’t get a copy). :crying: My husband was given a little card by the Doctor who did his medical in his foreign country who told him he would need that here, which we believe is his vaccination supplement (we aren't sure). To our grief, we can not find the little card. The I-485 package is almost ready to be files since everything is ready and we are only missing the vaccination supplement. How can we go about that? Does my husband have to get a whole new medical done?

Did he have the "card" with him at the foreign medical? If so, did the record any of that information?

BTW, as standard practice, no one is provided a copy of the "full" foreign medical... it is in the "A" file that he carried to the POE.

The doctor gave him the card when he did the medical in his country and he was told by the DR. that he would need it here, to keep it safe but when we were in the hassle of the wedding and moving i misplaced it. i did see the card but i really didnt take a look at it. i dont know what it said or if it had any info. i just saw the little card, asked him what it was and he told me it had to do w/ his vaccination.

Is that or was that the only record he had?

Did he have immunizations done at the exam or were they all done at various points in time throughout life?

That was the only thing he received, nothing else! and he says he received 2 shots the same day

If you don't have record it's not a big deal though. My wife also didn't have a vaccination record since it wasn't provided by the American embassy's panel physician in hers home country. So what we did, we just went through a Full exam that costs us $200 includes all the required immunization. First thing they do is to check what immunization the applicant has. If you don't have any immunization record then they simply test the blood and find out what it is immune too. For instance my wife was immune to MMR, Td, Hepitatius B. Dr just put in that info on the form. Good Luck.
