Check the box right next to 8. (the first box and only box on this line)

Check the box Yes on the second line of section 8.

Section 9.

If you are sponsoring another one (your daughter, son etc who will follow the fisrt one), you should check this one. Otherwise, leave it unchecked. Good luck
