Printing DS-3032 from website

My printer cannot print properly the DS-3032 I downloaded from NVC website. Did anyone else have this problem or is it specific to my printer or computer? Any trick to print this Pdf properly?

The only thing that the printer print is the USA eagle... Very strange, never had a problem with any other PDFs..

Ok I managed by using another older printer, although the quality is not great...

I printed out the barcode using the info on VjWiki...

One question. HOW BIG is the bar code supposed to be? I printed from the pop-up and then by copying and paste and I got two different sizes. Does it matter for the barcode reader? The larger one is about 2.7" long..

I repeated the name and telephone no. of my wife under I APPOINT and NAME OF THE PERSON WHO WILL ACT AS AGENT.
