Questions about paperwork: Adjustment of Status for K1

Hi all,

My fiance is going to be coming to the US in a few weeks and we have begun looking into the adjustment of status paperwork. I am a little confused about two points and was wondering if someone could help to clarify for me.

1) The I-693, Medical Examination of Aliens Seeking Adjustment of Status. The notes about this on the site say that if "you are a K-1 fiancé(e) who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement (I-693A), not the entire medical report. You may include the vaccination supplement with your adjustment of status application."

Is this a form that is given by the medical examiner at the exam my fiance attended as part of his receiving the visa? I could not find a form by this name.

2) I-864, Affidavit of Support. Does this form and all the documentation have to be submitted a second time? Just with updated dates, etc.?

Some (including us), have been successful submitting the DS 3025 which is the vaccination supplement you can request from your physical during the K1.

07-28-2008 - Email from CRIS confirming CSC received case

08-25-2008 - Received AP in the mail (NO STATUS CHANGE OR CRIS EMAIL)

08-29-2008 - Received EAD card in the mail (NO STATUS CHANGE OR CRIS EMAIL)

09-22-2008 - Received Email from CRIS for I-485 "CARD PRODUCTION ORDERED!"

09-27-2008 - Received Green Card in the mail!

Phase 3....

06-29-2010 - Sent I-751 and package to Remove Conditions

07-06-2010 - NOA1

08-24-2010 - Biometrics
