I-693A Vaccination Supplement from foreign Panel Physician?

Hi there!

A couple of days ago I had my K-1 visa interview in Frankfurt, Germany. I had my medical prior to the interview and the doctor gave me the missing vaccinations. However, I did not receive any medical documentation nor the DS-2025 vaccination worksheet.

My questions:

Can I send the Panel Physician a blank I-693A form and ask him to complete it?

When I file for AOS, does USCIS accept an I-693A Vaccination Supplement from a foreign Panel Physican?

Or in other words: Does a foreign Panel Physician have the same status as a Civil Surgeon in the US?

Thank you!

... and by the way: Does the I-693A need to be in a sealed envelope?

My guess is " no " the panel physician can not fill the I 693A. Of course you can ask and the USCIS may never catch it if you do but it is a different doctor.

The DS 3025 should be in the package that they doctor sent to the consulate. It should follow you in the sealed packet you give at the POE. They should remove the DS 3025 and give it to you. That is what you give to the CS to till the I693A. There are usually a couple of injections that are outstanding. Especially the varicella which must be given twice.

If you have a K2 take the DS 3025 to your local health department. They will finish the shots for free. You will need this for the child to enter school.

15 Sept 08 Application returned erroneously K2 not within 90 day timeframe

17 Sept Mailed 3rd application with mountains of proof of error copied Senator Mel Martinez

09 October 4th application package sent. This time they said the app signature page was a copy

10 Oct Sent package again 4th time.

25 Oct Received another NOA for Wife with $625 for the amount with one year extension

30 Oct 2008 Still nothing for step daughter. Checks still haven't cleared the bank

24 November Checks finally cleared the bank

February biometrics for wife Nothing for daughter.

They should have included that form during your interview. I would say get a blank DS-2025 form and have the panel physician fill that out if possible. They do have the same status, but their authority is probably limited to filling out certain forms. I personally think if you get them to fill out the 693 it wouldn't matter, but again, if you have that ability I would just have them fill out a new DS-2025 form.

Well, ja I think you are right.. I mean, I could probably convince the panel physician to fill out the I693A.. but I am not sure if I should take the risk.. on the I-693A form it says "to be completed by civil surgeon only".. I don't wanna receive a RFE when I file for AOS..

So I'll probably take the save way and have a civil surgeon in the US transfer the vaccination documentation to a I-693A..
