AOS application

I got a question to ladies that had their husbands go thru Casablanca...Jamal and I are just about ready to file his AOS application and all but I realized that he was never given the DS 3025 which is a form that the doctor who examined him in Casablanca should have given to him....He did his medical in March of this year and I know that he doesn't need another one...but what are we to do about the form that he was not given...should we contact the doctor in Casa and ask about it? are we going to get REF for medical? I am lost at this point and hate to waste more time...Thank you for any responses and suggestions...

Chances are you will get an RFE for the medical.

I would try contacting the doctor. See if you can find someone who can go to the doctor's office, pick it up and mail it to you. I don't think the doctor will do it on his/her own.

Even with the vaccination supplemental form, you will need to go to a civil surgeon here to have a paper filled out. I am forgetting the form number. We paid $25 for the form. We also had to pay to have a varicella vaccine since that was not given in Morocco and we had no way to prove he had chicken pox.

I got the vaccination supplement form and will schedule an appointment with a civil surgeon this week but will he fill it out if I do not have the DS3025? I guess if Jamal's brother maybe goes to Casablanca once I contact that doctor would be able to get that form...I wonder why the doctor did not give him the form in the first place?

I don't think its been uncommon unfortunately for some of the Moroccan ladies to be in the same predicament as you. If I am not mistaken - one was told the paperwork was not given to the beneficiary, of course the dr's are supposed to provide a copy for the very reason of AOS! I think that member was able to get a copy by sending her brother in law to the dr's office and had them send it over.

The civil surgeon will not do that for you unless you have a whole medical done again.

Most people only get the vaccination supplement. Call around to several civil surgeons because they all have different policies. Some want to charge for the whole physical again. Find a civil surgeon willing to fill out the I-693A only (thanks Laura). This is all you need to submit for AOS.
