My fiance doesn't know the house number for at least one of the places she lived at in the last 5 years. She says she knows the street though. Is that acceptable, or will this cause us a problem?

I'm sure that will be fine. It's not like they are going to go door to door and check! Just put as accurate information as you can. That's the main thing.

Sounds pretty typical for Thailand. Honesty is always the best policy. If you feel anything needs to be explained you can always attach an extra sheet of paper. I don't think my son ever knew the address of one of the places he stayed over there. The landlord didn't speak English, and at the time he didn't know Thai. He just put the name of the city for that time period.
