Which is the vaccination supplement form then???

Timeline

I was RFE'd a while ago for the vaccination supplement. Which I thought was the form I-693. But that form clearly states on top that it's for "Medical Examination of Aliens" and it doesn't seem to provide space for vaccination listing. The three civil surgeons I called already tried to convince me that I need that medical Examination (costing me $340 a pop). I don't wanna show up at their office with the form entitled "medical Examination". Can someone help me find the right form I'm supposed to bring with me to the civil surgeon's office, please? Thank you....

TIMELINE

April 27, 2004 I-29F sent to Vermont Service Center

May 4, 2004 'I-29F received' NOA

May 28, 2004 'I-29F approved' NOA

June 7, 2004 State Departments's National Visa Center confirms receiving approved I-29F petition

October 07, 2004 Interview

October 22, 2004 Arriving in the US

January 8, 2005 Married.

Timeline

did u have a medical examination in your country of origin?

This is what I done:

I had my medical in Birmingham, UK.

I gave the sealed envelope at my interview in the UK.

When I was applying for AOS it asked for Vaccination supplement, I phoned the medical center in Birmingham, UK and asked them to send me a copy of everything in the brown envelope.

When I recieved it, I sent it in with the AOS forms, I did NOT fill in an I-693 form, I also phoned homeland security and asked them, they said if I sent the medical exmaination results with the vaccination suppliment that I had in the UK, then I didn't need to fill out the I-693.

Had my green card interview, no problems.

Timeline

You may need to ring around a bit more.

Try the clinics catering for the Hispanic community.

They did mine for free, but you should only need to pay a small fee.

You have the right form.

You have the correct form, but you only need Appendix A filled out. Call around and get what you need. Tell the CS that you do not need to do a full medical.

I printed out the entire form this website links to and it does not include the Appendix A.

I'm about to start hurling my phone at random walls, so many time I've heard that I need the full medical. I'm afraid that if I bring the Medical Examination-entitled form, they'll go ahead and do that! I don't have $300+ to spare!

Anyhow - I found this: http://www.cdc.gov/ncidod/dq/pdf/ti-03/appdx-a_693vacc.pdf throught google search. Is that enough to bring with me or should I still bring the damn I-693?

Thanks guys. I've had my previously scheduled AOS interview cancelled recently, and not rescheduled, and my EAD's about to expire, my Advance Proles have been used up, I'm pretty fed up with the waiting and the money I've had to pour into this. And having to work two minimum-wage jobs instead of being a teacher (which is what I was doing in Poland) doesn't help my mood.
