i dont think so..the civil surgeon gives it in a sealed envelope and you are not allowed to open it..the civil surgeon gave me a copy for reference anyway..

Is it ok if we send zerox copy of I-693A--vaccination record form which we got while medical examination prior to K1 visa? or we have to send to original?

also want to confirm that I-864 -- affidavit of support does not need notarization?..does it?

You would not have received a I-693A from the foreign medical prior to the K-1... It would be a DS-3025...

I only submitted copy of DS 3025. I don't received RFE I already have GC interview schedule Nov. 25 next week.

I-693 part 2-5 needs to be filled in by civil surgeon. The vaccination record may be photocopied or not. it really doesn't matter... I-693 + vaccination record will be sealed in an envelope by the civil surgeon.

I-864 doesn't need to be notarized.

The I-693A form is out and the I-693 is revised. If your K1 medical was after June 1, you have to use the new form. There is a sticky at the top of this AOS forum about the vaccinations, but the information has changed so the beginning posts are out of date. :thumbs: The CS that did Claudeth's also put it in a sealed envelope. He did make a copy for us.

Means I have to go nearby Civil surgeon and ask him to transcribe on to I-693A form?

Well you could, but the I-693A was made obsolete in May 2008

Just to clarify...u sent xerox copy of DS-3025 or u sent original that u got while physical examination in ur home country?

You aren't "out of the woods yet. Peeps have been asked to get the I-693 at the AOS interview.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship
