If the interview letter has the forms that the beneficiary has to fill out, this is the packet 4. Packet 4 are the forms and the medical letter.

Packet 3 was a checklist of everything you need to bring to the interview. You signed it & sent it back acknowledging you understand what to bring.

There are no forms with packet 4. Packet 4 is simply the letter you have now telling you the interview date.

The medical forms will be given to you at the interview. You will do the medical portion & return those forms to Embassy prior to actually getting the visa. The visa is generated by computer processing & takes about 3 days minimum so there is time to review & or correct any minor problems with the medical if there are any.

Just review what you said you have as far as the documents required at the interview & make sure they are complete & correctly done. Then all you need to do is go to the interview you have waited all this time for.

Packet 4 should be the interview letter along with instructions on getting the medical done at least 2 weeks prior to the interview, passport and passport pics will need to be taken to the medical

I don't know about other consulates, but US Embassy Manila doesnt send out packet 4, they only send the interview letter. The packet 4 can be downloaded at the embassy website. Goodluck.

By the way, Packet 4 contains the forms that the beneficiary needs to fill out. And also instructions on what to bring to the interview. Its all found at the embassy website. Hope this helps.
